Important Announcement: First Eye Care Fort Worth Closure

After 30 years of proudly serving the Fort Worth community, First Eye Care, located at 3608 Altamesa Blvd, Fort Worth, TX, will permanently close on October 31, 2026.

It has been a privilege to care for our patients and their families over the years. We are incredibly grateful for the trust, loyalty, and relationships we've built throughout our time in the community.

Continuity of Care

Your ongoing eye health remains important to us. We encourage all patients to establish care with a new eye care provider to ensure there is no interruption in their vision and eye health needs.

If you need assistance finding a local optometrist, we recommend using the American Optometric Association's Doctor Locator.

Medical Records Requests

Your medical records will remain confidential and available upon request. If you would like a copy of your records or need them transferred to another eye care provider, please follow these steps:

• Complete the Records Release Request Form using the button below.

• Email the completed form to patientcare@firsteyecarefortworth.com.

• Please allow up to 4 weeks for your request to be processed.
